You start in the Nether, and can travel through a new dimension, the Abyss, to access things the Overworld would have had, such as Strongholds and Ancient Cities. You can fight others, build bases, and explore.
Lifesteal! Dying loses max health, but killing bosses and players gives you more! Minimum 5 hearts.
